Life Care Centers

Join Life Care Centers as an MDS Coordinator RN, leading the assessment and care planning process for residents in a senior living and rehabilitation setting. You will complete and manage MDS assessments, ensure accuracy and regulatory compliance, and collaborate with interdisciplinary teams to develop individualized care plans. This role supports quality improvement, drives reimbursement accuracy, and mentors nursing staff on documentation standards.
