- Conduct thorough evaluations of patients in both inpatient and outpatient environments while crafting personalized treatment plans
- Implement therapeutic physical therapy interventions that enhance mobility, strength, balance, and overall functionality
- Collaborate effectively with nurses, physicians, case managers, and other rehabilitation experts to maximize patient outcomes
- Educate patients and their families on therapeutic goals, discharge plans, and home exercise protocols
- Meticulously document evaluations, treatments, and progress in accordance with regulatory and facility guidelines
- Engage in multidisciplinary care rounds and conferences to ensure a smoothly-coordinated care experience
- Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution
- State licensure as a Physical Therapist
- Preferred CPR certification
- Specialized certifications (such as neurological or geriatric) will be regarded positively but are not required
- Previous experience is highly favored
- Adept in treating patients who face complex medical and mobility challenges
- Excellent communication and teamwork capabilities
- Proficient with digital charting and clinical documentation systems
