Physical Therapist (PT) - Outpatient Mobile

Role The Physical Therapist provides outpatient rehabilitation services in-home for older adults, assessing and developing personalized therapy programs to promote independence, mobility, and health. They evaluate patients, create treatment plans, and instruct patients and caregivers on therapeutic techniques. Responsibilities include supervising support staff, ensuring documentation compliance, making interdisciplinary referrals, and maintaining equipment. The role involves implementing various therapeutic modalities, fabricating adaptive devices, and performing mobility assessments. The therapist collaborates with the healthcare team to optimize patient outcomes and adapts plans based on individual needs. Qualifications A valid state licensure in Physical Therapy. A Master's degree in Physical Therapy or equivalent. Relevant experience or foreign credentials evaluated as equivalent. Must have reliable transportation and a valid driver’s license. Patient‑centered outpatient care in‑home environment. Develops and adjusts treatment plans. Provides education and training for patients and caregivers. Ensures safety and quality standards. Supervises support personnel as needed.
