About the Position

Hi,

We are actively hiring an experienced Home Health Registered Nurse (RN) for one of our clients for supporting home health services in Augusta. This role involves providing skilled nursing care to adult and geriatric patients in a home health setting while working independently and collaboratively with clinical teams.

You will be responsible for patient assessments, case management, documentation, and coordination of care in the field.

Job Title: Home Health Registered Nurse (RN)
Location: Augusta, GA 30907
Duration: 12 Weeks
Shift: Day Shift – 5x8 Hours (8:00 AM – 4:30 PM)
Pay Range: Weekly $2000– $2300-Travel (Local- $2000)

Responsibilities

• Perform Start of Care, routine visits, recertifications, and discharge assessments
• Provide case management and coordinate patient care plans
• Administer medications and manage IV therapies as needed
• Complete OASIS documentation accurately and timely
• Provide wound care, catheter care, and chronic disease management
• Educate patients and families regarding treatment plans and home care needs
• Maintain accurate documentation using HomeCare HomeBase (HCHB)

Pay Rate: Negotiable based on experience

Requirements

• Minimum 1 year of recent Home Health RN experience required
• Must have Home Health experience using HCHB (HomeCare HomeBase)
• Active BLS Certification required
• Strong communication and organizational skills required
• Ability to work independently in the field
