Housing Case Manager

Plymouth Housing

Plymouth Housing is seeking a Housing Case Manager to support residents in our permanent supportive housing program. You will work on an onsite team to develop Housing Service Plans, coordinate care, and advocate for residents, promoting housing stability and well-being in a harm-reduction, trauma-informed framework. The role involves coordinating with internal teams, maintaining documentation, and assisting residents with appointments and transportation as needed. #J-18808-Ljbffr
