Medical Social WorkerThe Medical Social Worker provides medical social services to clients in their homes in accordance with physician orders and under the direction and supervision of the Director of Nursing or other appropriate supervisor.Responsibilities of Position:Assesses the client's potential to cope with his/her social and health problemActs as consultant to the members of the health team; helps the team understand the social, emotional and environmental factors related to the client's health problemHelps clients to utilize the resources of their families and the communityProvides rehabilitative and supportive careAssists clients and their families cope with personal and environmental difficulties which might predispose them to illness or might interfere with their ability to obtain maximum benefitsAssists in the client's admission process to the Agency by performing the initial evaluation and assessment of the client's psychosocial status as well as an assessment of the client's home and family to identify socioeconomic and emotional factors that will affect the treatment planAssists in development and implementation of the interdisciplinary client care plan as it pertains to medical social workObserves, records and reports changes in the client's emotional and social factors that affect the client's illness and his/her need for care as well as his/her response to treatmentConsults with the attending physician concerning alteration of the plan of treatmentEnters clinical notes into EMR within required hours of scheduled visit (24 hrs. for non-Oasis visits / 48 hrs. for Oasis visits) and progress notes and other clinical record forms outlining the services renderedEvaluates the client's and family's responses to, and effectiveness of, the medical social work interventionConfirms, on a weekly basis, the scheduling of visits with the Director of Nursing to facilitate coordination of other staff visitsParticipates in Agency activities and committees that include staff development and in-service education when appropriateSupervises the social work assistant once monthParticipates in discharge planAssumes responsibility for self-development by continually striving to improve his/her Medical Social Worker practice through formal education, attendance at workshops and conferences, active participation in professional and related organizational meetings, and individual research and readingComplies with Agency dress code policyMaintains efficient flow of Agency work by reporting to work at assigned or scheduled times; follows procedures for notification of absence or time off requests.Maintains current TB testing (refer to Health Screening Policy).Job Conditions:Position requires the ability to communicate well orally and in writing. On occasion, may be required to bend, stoop, reach and move client weight up to 250 pounds and to lift and/or carry up to 30 pounds. Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the client's condition; to perform and demonstrate client care; and, to assess a client's home that may not be wheelchair accessible.Qualifications:Masters Degree from a School of Social Work approved by the Council of Social Work Education.Two years' experience with at least one year of experience in a healthcare setting (hospital, clinic, rehabilitation center, etc.)Compensation Range:$18.00 to $27.00The anticipated base salary range represents the Company's good-faith estimate of the compensation it reasonably expects to pay for this position at the time of posting. Actual compensation will be determined based on factors including experience, skills, qualifications, geographic location, internal equity, and business needs.
