About The Role Oversee the planning, implementation and evaluation of education services at the Diabetes and Nutrition Center at Northwest Hospital while also directly providing quality clinical education to patients within the center. Acts as Quality Program Coordinator, maintaining ADA Education Recognition requirements for both Northwest Hospital and multi-sites. Employs a mastery function to advise and teach other patient care associates; evaluates care delivered; performs advanced patient care functions. Key Responsibilities Provide Diabetes Self-Management Education (DSME) and Medical Nutrition Therapy (MNT) in the center. Provide leadership and direct supervision while seeing patients as a clinical coordinator. Conduct accurate and complete DSME and/or MNT assessments. Identify and prioritize patient/family needs. Develop DSME and/or MNT care plan to meet patient needs and standards of care, provide instruction on the plan in clinic and virtually. Employ advanced skills providing evidence-based care for diabetes and other disease states. Provide and develop appropriate educational materials and topics to support learning and growth of patients and/or families. Update and develop department resources/educational materials to reflect current evidence-based research and practice. Oversee planning, implementation, and evaluation of DSMES at all sites, maintaining ADA recognition and participating in evaluation. Maintain ADA recognition, participate in evaluation of DSMES effectiveness, including outcomes tracking and continuous quality improvement. Coordinate identification of DSMES stakeholders, develop education for diabetes program and community, liaise between team, stakeholders, other departments, and administration. Provide leadership and direct supervision of diabetes and nutrition team, delegate tasks accordingly. Actively participate in performance management process and professional development for the team. Monitor and facilitate maintenance of DSME team members qualifications (CE credits, training competency, licensure, registration). Monitor Diabetes Center metrics for program growth and continuous improvement opportunities. Identify opportunities and make recommendations for process improvements resulting in optimized patient care and center efficiency. Track outcome measures for ADA recognition. Participate in cost containment measures, utilize supplies efficiently within budget, provide input for budget development, accurately record charges, execute efficient patient treatment, and use work time productively. Work collaboratively with other hospital and LBH departments to advocate for and ensure patient care needs are met, foster community engagement, ensure compliance with hospital regulations. Demonstrate continuous professional growth through continuing education credits for licensure and advanced certifications, promote ongoing learning. Keep up to date with practice changes. Utilize evidence-based, best practice guidelines, stay up to date with new and improving technologies for diabetes management including medication options, CGM, CSII. Develop and maintain nutrition and diabetes management practice knowledge, motivational interviewing skills, RDN/LDN and CDCES registration and continuing education requirements. Qualifications Education: Bachelor's degree in nutrition or related field. Experience: 3-5 years. Certification/Licensure: Registered Dietitian (RDN).
