RN Case Manager Requirements

• Minimum 2 years of inpatient RN case management experience required
• Active Indiana RN license or compact RN license required
• Strong hospital utilization management and utilization review experience required
• Proficiency with InterQual and MCG medical necessity criteria required
• Experience with Epic documentation preferred
• Ability to work independently and prioritize tasks effectively
• Strong collaboration and communication skills with interdisciplinary care teams
• Case management certification preferred

RN Case Manager Responsibilities

• Complete comprehensive patient assessments to support status review, discharge planning, and care coordination
• Drive progression of care and facilitate efficient transitions to the next level of care
• Collaborate closely with the care team to support utilization review, patient throughput, and patient outcomes
• Document thoroughly, accurately, and in a timely manner within Epic EHR
• Manage primary unit assignments while providing coverage to other units as needed
• Support inpatient case management functions across various hospital units
• Identify priorities independently and follow through on assignments without prompting
• Maintain professional standards and contribute to a patient-centered care environment
