Position Summary

The Care Coordinator is often the first point of contact for prospective clients and plays a vital role in creating an exceptional customer experience. This position is responsible for answering inquiries, conducting intake calls, coordinating caregiver schedules, supporting client onboarding, and ensuring clients receive outstanding care. The ideal candidate is warm, professional, detail-oriented, and able to confidently communicate with families during important healthcare decisions.

 

Responsibilities

• Answer incoming calls from prospective clients, families, referral sources, and caregivers with professionalism and compassion.
• Conduct intake calls to understand clients’ needs and educate families about our home care services.
• Coordinate and maintain caregiver schedules to ensure appropriate client coverage.
• Assist with onboarding new clients, including gathering documentation and coordinating service start dates.
• Maintain accurate client and caregiver records within our scheduling software.
• Communicate regularly with clients, families, caregivers, and referral partners to ensure a positive care experience.
• Monitor ongoing client satisfaction and assist in resolving scheduling or service concerns promptly.
• Collaborate closely with the office team to support daily operations and maintain exceptional customer service.
• Assist with administrative projects and other office responsibilities as needed.
• Maintain compliance with company policies, state regulations, and documentation requirements.

 

Qualifications Required

• Minimum of 2 years of experience in a customer service, healthcare, scheduling, or care coordination role.
• Exceptional verbal and written communication skills.
• Strong organizational skills with the ability to manage multiple priorities simultaneously.
• Ability to remain calm, compassionate, and solution-focused in a fast-paced environment.
• Proficiency with Microsoft Office and the ability to learn scheduling software quickly.
• Reliable transportation for occasional client or community visits.

 

Preferred

• Experience working in home care, home health, hospice, senior services, or a medical office.
• Previous experience with client intake, scheduling, staffing coordination, or caregiver management.
• Knowledge of Nevada home care regulations is a plus.
• Experience using WellSky or similar healthcare scheduling software.
