Home Health Care Manager - Flexible Work & Family Wellness

Cardinal McCloskey Community Services (CMCS) is seeking a Home Health Care Manager to coordinate and oversee comprehensive care management for at-risk children and families. You will advocate for clients, work with schools, healthcare providers, and community partners, and ensure person-centered care tailored to each family’s needs. Responsibilities include initial assessments, Plans of Care, service coordination, documentation in EHR/MAPP, and connecting families with Medicaid, SNAP, housing, #J-18808-Ljbffr Cardinal McCloskey Community Services
