Job Description
Registered Nurse - Case Manager

Schedule

Day shift, Monday through Friday, 8:00 AM to 5:00 PM, with some Saturdays required.

Assignment Details

2-3 years of case management experience required.

Requirements

• Registered Nurse (RN) license required.
• BLS certification required.
• Ability to work with technology (typing, Microsoft, Epic).
• Excellent written and verbal communication skills.
• Ability to work in a fast-paced environment while organizing and prioritizing work.
• Experience assisting adults with home health setups and discharge processes.

Responsibilities

• Manage case loads and coordinate care for patients to ensure discharge plans are met.
• Collaborate with interdisciplinary teams to facilitate patient needs and resource access.
• Assist patients in navigating home health services.
• Maintain accurate documentation and reports of patient cases in the electronic medical record (EMR) system.
• Monitor patient progress and outcomes, making adjustments as necessary.
• Educate patients and families on care plans and available resources.
