Physician, Geriatrician (Santa Rosa, CA) Opportunity Information Sutter Medical Group of the Redwoods (SMGR) is recruiting a Board-Certified Geriatrician to join our established Geriatric practice within Sutter Senior Care. This is an exciting opportunity to practice in a team-based, value-driven outpatient setting focused on delivering high-quality care to older adults, particularly those with complex medical and social needs. The ideal candidate will have experience in a geriatric group practice or multi-specialty outpatient setting, with a strong background in primary care geriatrics and interdisciplinary team-based care. Position Highlights Established Senior Care clinic model within a value-based care framework Focus on complex, vulnerable Medicare populations Collaborative multi-disciplinary care teams Opportunity to impact quality, outcomes, and patient experience Mission-driven environment supporting healthy aging and dignity in care Outpatient-only setting Monday through Friday clinic schedule Shared call with physician and APC team Emphasis on value-based, panel-oriented care Key Responsibilities Provide comprehensive outpatient care for geriatric patients Diagnose and manage chronic and complex conditions (e.g., diabetes, hypertension, heart disease, cognitive disorders) Deliver preventive care, screenings, and immunizations tailored to seniors Manage polypharmacy and medication safety Collaborate with APCs, nurses, social workers, and specialists to deliver coordinated care Coordinate referrals and support care transitions across settings Educate patients and caregivers on disease management and healthy aging Participate in team huddles, quality initiatives, and value-based care programs Qualifications MD or DO; Board Certified in Geriatrics, Internal Medicine, or Family Medicine Valid and unrestricted California medical license (or eligibility) Experience in outpatient geriatrics or primary care preferred Experience in interdisciplinary, team-based care models preferred Strong clinical, communication, and patient engagement skills Passion for caring for older adults and underserved populations Join Us and Enjoy Competitive base salary aligned with primary care compensation: $310,000 - $340,000 Potential Incentive bonus up to $50K per Year {tied to quality and performance metrics} Comprehensive benefits package: Health, Dental, and Vision Benefits include paid CME, paid time‑off, pension plan and 401(k), life insurance, short‑term and long‑term disability insurance, and cell phone reimbursement Malpractice and tail insurance coverage Relocation allowance and sign‑on bonus Retirement program and additional incentives available Organization Details Like the majestic redwoods that surround us, we rise tall above the fog.
