RN Care Manager

Position: RN Care Manager, responsible for assessing patient needs, developing care plans, coordinating services, and facilitating communication among patients, families, and healthcare providers. Conducting comprehensive patient assessments of medical, social, and psychological needs. Collaborating with healthcare teams to create tailored care plans. Identifying barriers to care and coordinating appropriate services. Managing patient transitions, referrals, and follow-up appointments. Providing education on diagnoses, treatment options, and self-management strategies. Maintaining accurate electronic health records and documenting interactions. Participating in quality improvement initiatives and ensuring compliance with healthcare regulations. Key duties include: Conducting comprehensive patient assessments of medical, social, and psychological needs. Collaborating with healthcare teams to create tailored care plans. Minimum of 2 years clinical nursing experience, preferably in acute or hospital settings. Current RN license in Illinois. Preferred certification as a Case Manager (CCM) or similar. CPR/BLS certification for healthcare providers. Strong assessment, communication, and collaborative skills.
