About the Opportunity

We are building a physician-led group dedicated to bringing high-quality, on-site pain management care to residents of skilled nursing and long-term care facilities. As we launch this service line, we're looking for a Physician (MD), preferably board-certified or board-eligible in Pain Management, to join as a founding provider and help shape the clinical model, protocols, and growth strategy from the ground up.

This is a rare opportunity to combine clinical practice with the chance to build something new: you'll deliver direct patient care while also having a voice in how the program is designed, how it scales to new facilities, and how the group grows over time. Founding team members will be positioned for expanded leadership, equity, or partnership opportunities as the practice grows.

What You'll Do

• Provide comprehensive pain management evaluation and treatment for residents at partner nursing homes and long-term care facilities, both in person and via scheduled facility visits.
• Develop individualized, multimodal pain management plans (pharmacologic and non-pharmacologic) appropriate for a geriatric, medically complex population.
• Perform interventional pain procedures where clinically indicated and within scope (e.g., injections), coordinating with facility staff for pre- and post-procedure care.
• Collaborate with facility medical directors, nursing staff, and interdisciplinary care teams to integrate pain management into residents' overall care plans.
• Manage medication regimens with careful attention to polypharmacy, fall risk, cognitive status, and regulatory requirements around controlled substances in the long-term care setting.
• Maintain thorough, timely documentation and ensure compliance with facility, state, and federal regulations governing nursing home care.
• Serve as a clinical resource and educator for facility staff on pain assessment and management best practices.
• Participate in building out clinical protocols, quality metrics, and best practices as a founding member of the group.
• Contribute to the group's growth strategy, including onboarding of new partner facilities and, over time, potential mentorship of additional providers joining the team.
• Develop and implement a training program for new hires.

Qualifications

• MD or DO with an active, unrestricted medical license (or eligibility to obtain licensure in the relevant state).
• Board certification or board eligibility in Pain Management strongly preferred; backgrounds in Physical Medicine & Rehabilitation, Anesthesiology, Geriatrics, Family Medicine, or Internal Medicine with significant pain management experience also welcome.
• Experience with or strong interest in geriatric care and the long-term care / nursing home setting.
• Valid DEA registration and ability to manage controlled substance prescribing in accordance with state and federal guidelines.
• Comfort working across multiple facilities and coordinating with varied care teams.
• Strong communication skills with patients, families, facility staff, and interdisciplinary partners.
• Entrepreneurial mindset and interest in helping build a new service line, not just staffing an existing one.

Location & Schedule

Facility visits across our partner nursing homes in the service area; schedule and visit cadence to be determined based on facility census and provider availability. Flexible/part-time arrangements may be considered for the right candidate, with potential to grow into a full-time or leadership role as the practice expands.
