Licensed Practical Nurse

Steuben Center for Rehabilitation and Healthcare is seeking a dedicated Licensed Practical Nurse (LPN) to join our Healthcare & Medical Services team. In this role, you will provide hands‑on nursing care to residents in a skilled nursing and rehabilitation setting, supporting our mission to enhance patient outcomes and improve quality of life. Working under the direction of a Registered Nurse and in collaboration with our interdisciplinary care team, you will deliver compassionate, patient‑centered care while adhering to high standards of clinical quality, safety, and regulatory compliance. As an LPN at Steuben Center, you will perform direct nursing care including monitoring vital signs, administering medications and treatments, performing wound care, assisting with respiratory and tracheostomy care as appropriate, and providing support with activities of daily living. You will carefully observe residents for changes in condition, accurately document all care and interventions in the electronic medical record, and promptly report any significant findings to the supervising nurse or provider. Your clinical judgment and attention to detail will help ensure early identification of complications and support positive rehabilitation outcomes. You will contribute to individualized care plans by offering input based on your ongoing assessments and interactions with residents and their families. In collaboration with RNs, therapists, physicians, and social services, you will help coordinate care that supports both short‑term rehabilitation and long‑term resident needs. You will provide patient and family education on treatments, medications, and health management strategies, always maintaining a compassionate and respectful approach that honors each resident’s dignity, preferences, and cultural background. In this position, you will consistently follow infection control practices, fall prevention protocols, and all facility policies and procedures.
