Street Outreach Case Manager (Stationary) - 3rd Shift

Please note that this position will be working in Mesa, Arizona. The position requires Sunday–Thursday, 3:00 PM–11:00 PM availability and includes a $3 shift differential. Your Top Responsibilities Help oversee the daily operations of an assigned homeless day relief center, where people experiencing homelessness can receive relief and resources. Connect with and motivate people living on the streets to strive toward self‑sufficiency. Ensure all client‑level and other program data are properly recorded. Assist homeless people in navigating the social service system to identify resources that will help end their homelessness. Set goals and develop an individualized case plan with each outreach prospect. Establish and maintain relationships with community stakeholders and attend community outreach events. Ensure that client information is entered timely and accurately in the PRM database. Lead or participate in devotions, prayers, or other organization activities that promote a credible Christian witness and lifestyle. Requirements What You Should Have Bachelor’s degree in Social Work, Sociology, Psychology, Counseling, or another social service field, or 4 years of experience in case management, social work, or a similar setting, or a combination of education and experience. Certified in Peer Support, preferred. Current valid Arizona driver’s license with a clean driving record of 2+ years (as verified by an MVR check) and the ability to qualify for the company insurance policy. Knowledge of the problems facing the street homeless population and awareness of resources that help people exit homelessness. High composure and ability to remain calm in stressful situations. Discernment and understanding of the importance of keeping client information confidential. Excellent communication skills. Proficiency in Microsoft Office apps.
