Key Responsibilities:

- Support assigned consumers with routine daily care to promote independence in accordance with their recovery plans.

- Use written instructions and modeling techniques to facilitate the development of independent living skills.

- Familiarize yourself with specific direct care interventions outlined in treatment plans.

- Assist consumers with activities of daily living, including personal hygiene, mobility, and medication management.

- Monitor and document consumer progress, reporting any concerns or changes to the supervising team.

- Maintain a clean, safe, and supportive environment for all consumers.

- Collaborate with multidisciplinary team members to ensure comprehensive care delivery.

Skills and Qualifications:

- High school diploma or equivalent; additional training in healthcare or rehabilitation services preferred.

- Compassionate, patient, and respectful attitude towards consumers.

- Strong communication and interpersonal skills.

- Ability to follow detailed instructions and adapt to individual needs.

- Basic knowledge of safety procedures and infection control practices.

- Prior experience in a healthcare, rehabilitation, or social services setting is a plus.

- Ability to work effectively both independently and as part of a team.
