Direct Support Professional (DSP)/Day Program Facilitator

Day Program FacilitatorThe Day Program Facilitator provides direct support to clients who participate in the Day Program including developing and facilitating groups, as well as, individual support. The facilitator documents all information required to determine the client's progress towards the achievement of group and individual participant goals.Essential FunctionsCreate and facilitate groups that help clients achieve goals and outcomes identified in their Individual Service PlanUnderstand and follow client's Individual Service Plan to ensure excellent service provision; provide assistance to program participants as neededTransport clients as assigned (able to drive a 15-passenger van)Foster the growth of individuals by teaching and offering opportunities for choice, initiation, and risk-takingMake preparation and serve lunch for attendeesProvide crisis intervention, conflict resolution, and mediation as needed.Participate in all staff meetings, trainings and client meetings as assigned.Must check email and voicemail dailyUtilize Electronic Health Record System to create accurate and timely notes on all services delivered and relevant client information that meets or exceeds agency and state standardsComplete other departmental documentationCorrect all documentation as directed by supervisorAttend all electronic health record (Foothold) training as requiredCommitment to Core ValuesAdhere strictly to HIPAA requirements and all agency confidentiality guidelines.Establish positive, balanced relationships with clients, as demonstrated by:Treatment of individuals with respect and dignity and in an age and culturally appropriate manner;A commitment towards the individual's rights and responsibilities; andEstablishment of clear boundaries that allow for productive interactions.CompetenciesPutting People FirstBuilding and Maintaining Positive RelationshipsDemonstrating ProfessionalSupporting Good HealthEnsuring Safety Both on Site and In the CommunitySupporting Independence and GrowthSkills and AbilitiesThe ability to communicate clearly both orally and in writingFlexible and able to work in a fast-paced environmentSelf-starter who can innovate new ideas and strategies for helping clients reach goals and outcomesPatience when working with individuals across a spectrum of intellectual and developmental disabilitiesConflict resolution skillsComputer SkillsEducational RequirementsHigh School Diploma or GEDAssociate degree or further degree in psychology, mental health, or a related field preferredWork Experience RequirementMinimum of two years working with individuals with developmental disabilities, or an equivalent combination of education and experience that provides the knowledge, skills and abilities to perform job duties and responsibilities proficiently.Position Type/Expected Hours of WorkHourlyMust be available Monday through Friday 8am – 5pm.Valid New Jersey Driver's license is required in good standing.Special Working Conditions/Physical/Requirements – Working EnvironmentThis job operates in a professional environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.Must be available for coverage on some weekends and after 5pm when required.Must hold a high school diploma.Valid New Jersey driver's license is required.Must hold CPR/First Aid Valid CertificationSuccessful clearance of NJ background check.Successful clearance of Drug ScreeningHas fulfilled the necessary training requirements and is in good standing in order to bill Medicaid for Support and Community Care Waiver (CCW) Services and CDSPhysical DemandsThe physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.While performing the duties of this job, the employee is regularly required to talk or hear.
