Position Summary

The Hospice CNA provides personal care services to hospice patients under the supervision of a Registered Nurse and in accordance with physician orders and individualized care plans. This role focuses on enhancing patient comfort, promoting dignity, and supporting families during end-of-life care.

Essential Responsibilities

• Provide personal care assistance, including bathing, dressing, grooming, toileting, and mobility support.
• Observe and report changes in patient condition, physical status, and emotional well-being to the hospice care team.
• Document patient care activities and relevant observations accurately and timely.
• Follow established hospice care plans and nursing instructions.
• Assist patients in maintaining comfort and quality of life.
• Support family members and caregivers through compassionate communication and care.
• Maintain regular communication with the RN, Clinical Coordinator, and interdisciplinary hospice team.
• Participate in staff meetings, training programs, and continuing education opportunities.
• Complete scheduled patient visits in accordance with agency standards and patient needs.

Qualifications

• Current Virginia Certified Nursing Assistant (CNA) certification required.
• Valid driver's license, reliable transportation, and auto insurance.
• Ability to travel to patient homes throughout the service area.
• Strong communication and interpersonal skills.
• Compassionate approach to caring for patients and families facing end-of-life challenges.
• Hospice, home health, or long-term care experience preferred.
