Location Hardin County Schools Healthy Kids Clinic Duties / Responsibilities Uses age‑appropriate assessment and intervention skills. Able to use a range of interventions, depending upon the needs of the patient/family. Assists members in developing cost‑effective, appropriate health care plans through optimizing members’ personal resources as well as community resources and programs. Promotes multidisciplinary care that is patient‑centered and considers all aspects of members’ personal, psychological, economic, and cultural needs. Maintains thorough knowledge of CFMC services/benefits, public/private community resources, as well as federal and state laws affecting health care planning. Completes required documentation of services and plan of care per policy. Participates in program development, orientation, and educational activities which further members’ capacity to better direct their own health care experience. Other duties as assigned. Qualifications One (1) year of experience in mental health, hospital, medical outpatient clinic, FQHC, home health, or long‑term care setting. Three to five (3–5) years of experience in a related environment. Master’s degree, licenses, certifications, or registrations as required. Current unrestricted license in the state in which one practices, required upon hire. Basic Life Support (BLS) for Health Care Providers. National Provider Identifier (NPI) required prior to employment start date. Key Skills Three to five (3–5) years of professional experience in a hospital or medical outpatient clinic, FQHC, home health, or long‑term care setting. Expertise with brief counseling interventions. Demonstrated ability to provide leadership. Advanced knowledge of local and national resources related to behavioral health care. Schedule Requirements Full‑time position, 5 days per week, 100% clinic‑based, in person work required. Work schedules are set at the discretion of CFMC and may be adjusted at any time based on the operational needs of the organization. Compensation Negotiable based on experience.
