Palliative Care Nurse Navigator - Emergency Department Location Rutland, VT Facility Rutland Regional Medical Center Job Category Nursing Schedule - Shift - Hours Full-time w/rotating wknds & holidays - Day/evening shifts - Monday-friday 10am-6pm, rotating weekends and holidays This positionwillsupport the Emergency Department. The Palliative Care Nurse Navigator is a specialized registered nurse who guides patients and their families through serious or life-limiting illnesses. They coordinate care, triage referrals, facilitate goals-of-care discussions, provide education regarding disease processes, and help connect patients to community resources to improve their quality of life. Graduate of an approved school of nursing, B.S.N. preferred. 3 years of professional nursing experience. 2 years of palliative or hospice care preferred. Required Licenses/Certifications BLS Certification through American Heart Associations. Certified Hospice & Palliative Nurse (CHPN) or equivalent certification* *Or obtained within 2 years of hire. Required Skills, Knowledge, and Abilities Demonstrates safe knowledge of nursing practice skills at all times based on theory and consistent with policy and procedure. Demonstrates specialty knowledge and practice of nursing. Excellent interpersonal, written, and verbal communication skills. Ability to effectively communicate and relate information to individuals of varying ages from diverse socio-economic backgrounds. In depth knowledge of utilization management criteria, complex medical and surgical patients, reimbursement and regulatory requirements including CMS guidelines.
