Registered Nurse-RN-MDS Coordinator Schedule & Benefits Full-time, day shift, Float position Competitive compensation Comprehensive medical, dental, and vision benefits for eligible full-time employees Paid Time Off (PTO) Career growth and advancement opportunities within Complete Care at Monmouth Supportive leadership and collaborative team environment Registered Nurse-RN-MDS Coordinator Qualifications Associate's or Bachelor's Degree in Nursing Current, unrestricted New Jersey Registered Nurse (RN) license Previous long-term care experience preferred Experience as a Registered Nurse-RN-MDS Coordinator or MDS assessment experience in a skilled nursing setting preferred Strong clinical assessment, communication, and organizational skills Knowledge of federal and state MDS regulations Ability to work collaboratively with interdisciplinary teams Registered Nurse-RN-MDS Coordinator Responsibilities As a Registered Nurse-RN-MDS Coordinator , you will: Coordinate and complete accurate MDS assessments in accordance with federal and state guidelines. Partner with nursing, therapy, social services, and other departments to develop individualized resident care plans. Ensure documentation supports quality resident care, regulatory compliance, and reimbursement accuracy. Monitor assessment schedules and update MDS documentation as residents' conditions change. Participate in care plan meetings and contribute clinical expertise to improve resident outcomes. Educate and support clinical staff on MDS processes, documentation standards, and regulatory updates.
