The Case Manager is a compassionate, mission‑aligned professional who recognizes that lasting change begins by meeting a mother right where she is. Case Managers partner closely with each mom to define actionable, achievable steps forward—and to celebrate each milestone as a sign of her growth and resilience. Far beyond case documentation or service referrals, this role is about showing up consistently, nurturing trust, and helping mothers navigate crisis with dignity. Through collaboration with volunteers and community partners, facilitation of personalized care plans, and deep relational engagement, the Advocacy Case Manager is a key part of every mom’s redemptive path toward lasting stability and restored family life. Key Responsibilities The key objectives include the following areas of focus and management: Intake and Crisis Intervention, Resource Coordination and Support, Documentation and Communication, and Volunteer Management. Intake and Crisis Intervention Receive and manage assigned client referrals with intentional care, conducting comprehensive, strengths‑based assessments to understand each mother’s unique situation. Partner with clients to develop clear, realistic, and empowering action plans that address urgent needs while laying a foundation for long‑term stability. Provide responsive, compassionate crisis intervention, supporting moms through high‑risk situations and coordinating with professionals and systems to ensure safety and support. Uphold each mother’s dignity and confidentiality at all times, creating a safe and trusted space for her to engage in change. Resource Coordination and Support Respond creatively and practically to meet clients’ immediate needs, while equipping them with tools to work toward self‑defined goals.
