Coordinate with an interdisciplinary team to implement and adjust plans of care. Monitor patient status, manage the progression of the caseload, and address barriers to goals. Educate patients and families about care plans, medications, symptom management, and follow-up steps. Document clinical assessments, plans, and outcomes in the electronic health record; communicate with referring providers and care partners. Facilitate referrals, referrals to ancillary services, and appropriate post-visit resources to support safe transitions. Requirements Active Registered Nurse (RN) license (state‑specific or as required for practice). Relevant case management and clinical experience in an outpatient setting. Strong assessment, critical thinking, and communication skills. Ability to collaborate with an interdisciplinary team and coordinate care across services. Current CPR/BLS certification or ability to obtain. Education: RN degree (Associate Degree in Nursing or higher) as the foundational credential.
