Registered Nurse - Hospice

Job Description

Job Summary:
Provide compassionate, patient-centered hospice nursing care to individuals in various home and facility settings. Lead interdisciplinary teams to ensure personalized care aligned with patients' goals and needs.

Responsibilities:

• Assess patients' clinical conditions and develop individualized care plans.
• Educate patients and caregivers on disease management and symptom control.
• Administer medications and treatments accurately while monitoring outcomes.
• Collaborate with interdisciplinary teams to coordinate comprehensive care.
• Advocate for patients' personal goals of care and comfort needs.
• Provide compassionate emotional support to patients and families.

Qualifications:

• Possess a valid RN license in the state of practice.
• Demonstrate strong clinical assessment and critical thinking skills.
• Have experience or training in symptom management and end-of-life care.
• Exhibit excellent communication and patient education abilities.
• Able to work independently with a flexible schedule including on-call coverage.
• Comply with healthcare regulations and hospice standards of care.
