Veteran Case Manager II - Housing & Independence

U.S.VETS

U.S.VETS is seeking a Case Manager II to support veterans in achieving independence through comprehensive case management. You will assess needs, coordinate services, document progress, and collaborate with VA and community partners to ensure housing, employment, education, and health goals are met. The role emphasizes HIPAA-compliant data handling, timely intake and discharge processes, and empowering veterans across transitional housing, workforce, SSVF, and aftercare programs. #J-18808-Ljbffr U.S.VETS
