Registered Nurse Home Health

Job Summary:
Provides comprehensive home health nursing care by coordinating and executing patient care plans based on physician orders, agency policies, and nursing best practices. Responsible for supervising home health aides and LPNs/LVNs while ensuring quality patient outcomes through competent clinical assessment and intervention.

Responsibilities:

• Evaluate patients’ health status and develop individualized care plans.
• Administer medications and treatments as prescribed by physicians.
• Supervise and evaluate the performance of home health aides and LPNs/LVNs.
• Document patient interactions and care activities accurately within the required timeframes.
• Communicate effectively with patients, families, and interdisciplinary team members.
• Travel to patient homes within designated territory, often up to 75 miles radius, ensuring timely care delivery.

Qualifications:

• Hold a current, valid RN license in the state of practice.
• Possess at least 1 year of recent adult home health or hospice experience.
• Be proficient in using HomeCare HomeBase (HCHB) EMR system within the last 6 months.
• Demonstrate competency in OASIS documentation if applicable.
• Have reliable transportation, a valid driver’s license, and agency-required liability insurance.
• Be able to travel up to 75 miles from the assigned clinic location.
