Senior Intensive Care Coordinator for Youth Mental Health

The Home for Little Wanderers in Somerville, MA is seeking a Senior Intensive Care Coordinator to join the management team and ensure individual care coordination services are delivered to MassHealth youth with serious emotional disturbances. You will oversee supervisees, participate in planning, and act as a liaison with collateral agencies.
