This location will offer medical innovations on a campus designed around whole person health with comprehensive services, destination lifestyle programs, and much more that will shape the next generation of health care. Our services include emergency services, urgent care, a wide range of medical specialists (including primary care), lab, imaging, orthopedics, sports medicine and rehabilitation. Responsibilities Monitors the recovery of discharged patients through the Patient Call Back Program. Calls discharged patients at designated times to ensure patient satisfaction and compliance with discharge education. Assesses patient knowledge, compliance, and understanding of discharge instructions through outbound calls. Identifies gaps or barriers in the discharge treatment plan and communicates effectively with the multi‑disciplinary team. Provides and reinforces patient and family education as planned by the discharge team.
