Dietitian - Chronic Disease & Weight (Hybrid/Telehealth)

Allaire Health Services, LLC is seeking a Registered Dietitian (RD) to join our interdisciplinary primary care team. You will provide Medical Nutrition Therapy (MNT) and Behavioral Counseling for Obesity to patients with diabetes, prediabetes, obesity, and other chronic conditions, in outpatient, SNF settings, and via telehealth for continuity of care.
You’ll work closely with physicians, nurses, and behavioral health professionals, document care in EMR, and support chronic disease management
