Responsibilities:

• Coordinate and conduct initial and comprehensive MDS assessments for residents, ensuring timely and accurate submissions to CMS

• Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team

• Identify and resolve discrepancies in MDS data, documentation, and care planning

• Collaborate with healthcare teams to identify and address resident needs, providing education and guidance as needed

• Maintain accurate and up-to-date records, including MDS assessments, care plans, and resident data

• Ensure compliance with federal, state, and local regulations, as well as organization policies and procedures

• Participate in quality improvement initiatives and contribute to continuous quality improvement efforts

Requirements:

• 2 years of experience preferred

• High school diploma or equivalent required; degree in a related field (e.g., healthcare administration, nursing) preferred
