Shifts: 4 x 10-hour shifts, 6:00 AM - 4:30 PM


Seven Healthcare are seeking a dedicated Home Dialysis Registered Nurse (RN) to join the supportive home dialysis program in Cooperstown, NY. This travel role specializes in home dialysis, providing a mix of clinic-based care, patient education, and clinical autonomy. Candidates will work with a specific patient group, focusing mainly on Peritoneal Dialysis and Home Hemodialysis treatments, and are expected to travel between clinic locations as part of their responsibilities.

Key Responsibilities of the Home Dialysis Registered Nurse (RN)

• Manage outpatient home dialysis clinics
• Conduct patient assessments and ongoing clinical management
• Provide modality education and training for new home dialysis patients
• Troubleshoot and triage patient concerns and treatment-related issues
• Coordinate care and support patient independence in the home setting
• Occasionally assist inpatient nursing units with CAPD exchanges
• Complete initial home visits for new patients as needed

Requirements of the Home Dialysis Registered Nurse (RN)

• Availability for on-call rotation every other week
• Personal vehicle required for patient home visits
• Previous Home Dialysis experience strongly preferred
• Peritoneal Dialysis (PD) experience highly desirable
• Experience in patient education and training preferred
• Strong assessment, triage, and patient education skills
