Geriatric NP/PA: Impactful Senior Care Leader

TruHealth

TruHealth in Plainfield, IN seeks an advanced practice provider (NP or PA) to improve outcomes for long-term care residents through coordinated, proactive care. You will work with a team of providers and case managers within nursing homes or assisted living communities enrolled in Medicare Advantage plans.

Role requires independent judgement, collaboration with supervising physician, and delivering personalized care while meeting regulatory requirements.
