Develop individualized treatment plans focused on mobility, strength, balance, and fall prevention. Educate patients and family caregivers on safe mobility techniques. Maintain accurate, Medicare‑compliant documentation. Collaborate with the care team to support patient progress. Qualifications Current Hawaii Physical Therapist (PT) license. Minimum 1 year clinical experience (home health experience preferred). Strong documentation and communication skills. Ability to work independently in a home setting.
