Hospice Clinical Manager - per diem

Location: Riverside What You’ll Do Oversee daily clinical operations and ensure high‑quality patient care. Participate in patient eligibility decisions with physicians and referral partners. Ensure thorough assessments and appropriate clinical care plans at admission. Lead case conferences and support strong communication across the care team. Review electronic clinical records for quality and compliance. Coordinate staffing to meet patient needs and optimize clinician productivity. Assign patients by clinician territory and reduce service overlap. Partner with HR to anticipate staffing needs and recruit clinicians. Track and analyze clinician productivity data. Implement new clinical and regulatory guidelines with Quality Assurance. Document care accurately in the EMR. Participate in ongoing training and education. Adhere to infection‑control, HIPAA, and professional standards. Participate in the Hospice Administrator On‑Call rotation. Other duties as assigned. Qualifications Required: Graduate of an accredited nursing school. Active RN license in the state of practice. 2+ years RN experience (Hospice preferred). 2+ years in a supervisory role. Additional: Current CPR/BLS certification. Valid driver’s license and insured personal vehicle. Proficiency with Microsoft Office. Knowledge of state and federal home health regulations. Ability to read, follow, and document clinical instructions in an EMR. Strong leadership, communication, and coaching skills. Ability to work professionally with adult patients. Flexibility and strong problem‑solving abilities. Physical and mental stamina; required health clearances (including TB/PPD or chest X‑ray). Why Join us?
