Role Homes and facilities in East Kissimmee The Social Worker (SW) provides and coordinates a wide range of social services including psychosocial support to patients and their families. Qualifications Master’s degree or above in Social Work (CSWE accredited). SWs hired before 2009 with degrees in other fields are grandfathered in. SWs hired before 4/21/2019 with BSWs are grandfathered in. Bachelor’s degree may be considered depending on geographic area (Note: Home Health requires MSW). Employees working in Georgia require current Georgia LMSW or LCSW. Minimum of one (1) year of experience including social work/counseling internship in health care or hospital setting. Familiarity with and working knowledge of the concepts and practices of both adult and pediatric patients. Experience with hospice (death and dying) and palliative (serious illness) settings. Prior experience working with an EMR/EHR system. Experience in children, family, and group modalities. Ability to manage multiple priorities and handle emotional stress of the workload. Ability to remain calm, professional, troubleshoot, and intervene in patient/family crisis situations. High ethical standards in hospice mission, confidentiality, sensitivity, and flexibility. Excellent communication (verbal and written) and interpersonal skills. Teamwork and appropriate intervention skills. Valid driver’s license and automobile insurance per company policy. Capability to travel to off‑site locations. Physical ability to access various home, hospital, clinic, nursing home, and assisted living facilities. Competencies Satisfactorily complete competency requirements for this position. Responsibilities Perform and document psychosocial assessment. Empower patient/families to negotiate with resource providers and make decisions. Assist other clinical staff in understanding family dynamics and significant social emotional factors related to health problems. Assist in developing a plan to address issues in a non‑judgmental manner. Maintain documentation reflecting timeliness, completeness, accuracy, legibility, and clarity of goals and interventions. Consult and collaborate with other professionals to promote quality and continuity of patient/family care. Perform other duties as assigned. For Home Health: participate in individualized Plan of Care development and case conferences; observe clinical symptoms and report changes; anticipate barriers to pain and symptom management; identify differences between curative and palliative goals; provide education; respect patient wishes and stated goals. For Hospice: share joint responsibility with clinicians and volunteers in providing psychosocial support, advanced directive information, plan of care, and other social services; attend IDG meetings; assist in plan updates; facilitate understanding of social services and bereavement aspects; evaluate bereavement needs; make condolence calls; coordinate plan with bereavement staff. For Palliative Care: assist the team or facilitate patient‑family meetings for goal setting and advance care planning; provide emotional support for serious illness; coordinate educational outreach; act as liaison between outpatient, inpatient, and community services. Additional duties for Florida Keys & North Florida: evaluate bereavement care needs, provide grief guidance, facilitate bereavement support groups, attend memorial services, maintain bereavement documentation, recruit and train volunteers, participate in volunteer‑related committees and events, maintain volunteer records, provide feedback at IDG meetings. Additional Requirements Consent to drug and/or alcohol testing after a conditional offer. Ongoing compliance with the Drug‑Free Workplace Policy. Background screening through the Florida Care Provider Background Screening Clearinghouse after a conditional offer and ongoing eligibility.
