Bi-lingual Creole Case Manager/Social Worker

We are seeking a Bilingual Creole Case Manager to support patients in a community health clinic serving individuals living with HIV/AIDS and underserved populations. Responsibilities Manage a caseload of approximately 50 patients. Coordinate medical appointments, referrals, and follow-ups. Help patients understand care plans and treatment needs. Connect patients to resources (housing, food, transportation, benefits). Advocate for patients and remove barriers to care. Maintain accurate documentation in EHR systems. Collaborate with providers and community partners. Qualifications Bachelor’s degree in Social Work, Public Health, Psychology, or related field (required). Experience in case management, care coordination, or social services. Fluent in Haitian Creole (required). Strong communication and organizational skills. Comfortable working with patients living with HIV/AIDS and underserved populations. Experience with EHR systems and documentation. Schedule Must be flexible to work shifts within clinic hours of 7:00 AM – 7:00 PM, Monday–Friday. Compensation & Benefits $22/hour.
