Position Overview:

The Case Manager Registered Nurse (RN) is responsible for coordinating and managing patient care across the continuum to ensure high-quality, cost-effective outcomes. This role involves collaboration with interdisciplinary teams, patients, and families to facilitate efficient transitions of care and promote optimal recovery.

Key Responsibilities:

• Assess, plan, implement, and evaluate individualized care plans for assigned patients.

• Coordinate patient discharge planning and post-acute care arrangements.

• Work collaboratively with physicians, nurses, and other healthcare professionals to ensure continuity of care.

• Monitor utilization of healthcare resources to promote efficiency and cost-effectiveness.

• Serve as a patient advocate by supporting informed decision-making and ensuring access to necessary services.

• Maintain documentation in accordance with hospital and regulatory standards.
