Location 1738 South Tremont Street,Oceanside, CA, 92054,United States Base Pay $28.85 - $29.13 / Hour Job Category Counselors Employee Type FT Non-Exempt Required Degree 4 Year Degree Hours: 40 hours per week; Monday - Friday, 8:00 am to 4:30 pm. This is an on-site position and requires in-person attendance to perform the essential functions of the job. As a Discharge Planner, you will: Assess discharge needs of each person served and coordinate discharge planning activities to ensure continuing care supports are in place as needed Collaborate with the treatment team to meet discharge goals Assist with initial screening and sign in process to the program as well as provide crisis intervention, discharge planning groups and outreach activities to residents at the program on an as needed basis Represent the program/CRF within the community by developing linkages and relationships with other services providers Educate clients regarding supports and resources available 19 days of PTO per year Monthly contribution of $100 towards qualified student loan debt; access to Public Service Loan Forgiveness (PSLF) program for eligible individuals Choice of medical plans, with minimal employee cost/expenses and excellent coverage Company-paid life insurance and Long-Term Disability Tax-Free Flexible Spending for medical/dental/childcare expenses Free Employee Assistance Plan Employee discount program, including pet insurance plans Opportunities for advancement and growth CRF inspires and mentors the next generation of behavioral health professionals! Our Crisis Centers are Short Term Acute Residential Treatment (START) programs (Del Sur Crisis Center, Esperanza Crisis Center, Halcyon Crisis Center, Hawthorne Crisis Center , Jary Barreto Crisis Center, New Vistas Crisis Center , and Turning Point Crisis Center ) that provide intensive, community-based crisis residential services for adults throughout San Diego County. They deliver a unique alternative to psychiatric hospitalization to clients who are experiencing a psychiatric crisis of such magnitude so as not to be manageable on an outpatient basis. Their services are provided in a home-like environment that specializes in the psychosocial rehabilitation of each individual. CRF staff members work together to meet the needs of San Diego County’s residents who are experiencing mental/behavioral health challenges. Education : Bachelor's degree in Psychology, Social Work, or Counseling required Required Skills and Qualifications CPR/First Aid certification required and must be current throughout employment Minimum of 4 years of experience in a behavioral health field Extensive knowledge of San Diego resources for individuals with mental illness, substance use issues, and homelessness preferred Ability to maintain calm demeanor in crisis situations is essential This program is housed in a two-story building that does not have an elevator. Ability to navigate stairs is required. Must be able to pass criminal/background clearance Summary Provides psychosocial rehabilitation interventions and services in order to meet the clinical needs of clients. Assists clients in accessing and using medical care, community resources, family and other support services to achieve an optimal level of health and functioning in the community.
