In-Home Health Clinician - Nurse Practitioner or Physician Assistant (Full Time)- Wenatchee

Overview Signify Health, part of CVS Health®, is seeking a full‑time Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations to individuals in their own homes. The role aims to build trusted relationships and deliver personalized health evaluations. Responsibilities Conduct 45‑minute in‑person evaluations, including medication and medical history reviews, physical assessments, and diagnostic tests as ordered. Use an iPad and an intuitive clinical workflow for each visit. Provide diagnostic and preventive services such as spirometry, diabetic eye exams, bone density screening, etc. Offer virtual visits as needed. Travel locally and within licensed states, as required. Work with Signify Health as a clinician colleague, fulfilling the specified role requirements. Qualifications Active, unrestricted license(s) in the coverage area(s). Board certification (required). Multi‑state licenses (preferred). Qualified to cover primary locations in Wenatchee and surrounding area. Accepted as a Nurse Practitioner or Physician Assistant.
