About the Position

Job Title: Licensed Masters of Social Worker (LMSW) Profession: Social Services Specialty: Care Coordination Duration: 13 weeks Shift: Day Hours per Shift: 7 Experience: Experience with goals of care, end-of-life planning, inpatient hospital social work, discharge planning, and oncology preferred. License: Licensed Masters Social Worker (LMSW) required. Certifications: Master’s Degree in Social Work (MSW) required. Must-Have: - Strong communication skills - Knowledge of psychosocial assessment Description: This position involves a Monday to Friday schedule from 9 AM to 5 PM, including a 60-minute unpaid lunch break. The assignment will cover a full-time leave of absence. Candidates must have experience in goals of care and end-of-life planning and conversations. Inpatient hospital social work experience, particularly in oncology, is preferred. The successful candidate will need to demonstrate strong communication skills. Knowledge of psychosocial assessments is required. Responsibilities include conducting initial evaluations and coordinating services for home care and equipment following discharge. The role requires effective documentation of care delivery, plans, and goals, ensuring compliance with coverage criteria. Candidates should be able to provide crisis intervention and refer patients to appropriate resources. Experience with documentation reflecting patient responses and caregiver education is essential. Candidates should be familiar with advance directives and treatment options for substance use. The position requires participation in care conference activities and effective collaboration with interdisciplinary teams. Knowledge of cultural issues and strong management skills are beneficial. The candidate must adhere to the highest ethical standards in social work. This role will involve working with a patient population that includes individuals with leukemia and other blood cancers.
