Job Summary

The Manager of Case Management is responsible for overseeing case management services, ensuring the delivery of high-quality, cost-effective care to patients through effective case management processes. This role involves leadership, supervision, and coordination of case management activities to promote patient health and satisfaction.

Qualifications

Education: Bachelor's Degree in Nursing or Bachelor's Degree in Social Work accredited by the Council on Social Work Education or Bachelor's Degree in related healthcare field.

Experience: Three year's experience in Case Management or related experience. Leadership experience.

Licensure: Current RN licensure in appropriate state. Certification in Case Mgmt. or related field.

Additional Skills/Requirements Required: Strong quantitative and qualitative reasoning, problem-solving, analytic and critical thinking skills. Excellent written and verbal communication skills including the ability to write clearly and accurately in non-technical terms about complex policy. Ability to consult on matters related to policy in an effective manner. Demonstrated career progression with increasing roles of responsibility. Commitment to working in a team environment with an emphasis on collaboration. Ability to demonstrate initiative, ownership and accountability of mid-size projects. Ability to function effectively in an environment with multiple and fluctuating priorities; successfully manage multiple high priority projects simultaneously. Advanced Microsoft Office skills to include PowerPoint, Excel, and Word.

Works with physicians and all members of the Interdisciplinary team (when applicable) to develop or improve processes around the care of their patients. Serves as a patient advocate. Enhances a collaborative relationship to maximize the patient's and family's ability to make informed decisions.
• Human Resources and Organization Development: Provides ongoing and annual feedback to staff on performance consistent with organizational policies and procedures, including performance improvement plans. Coaches staff in developing plan to achieve personal career goals. Provides for education and training of staff. Builds teams and partnerships. Follows the process for open job posting and allows employees equal access to opportunities to develop themselves. Monitors, evaluates and coordinates workload trends and manages resources to meet unit needs. Coordinates planning for human resource needs based on productivity and workload. Continually seeks to implement activities that reduce staff turnover rate.
• Financial Management: Manages positions within the department to achieve quality and productivity, and meet budget. Provides input into the annual operations and capital budgets. Manages the department budget (measures, monitors, and balances human and material resources). Provides input and collaboration for strategic planning for system/facility and community.
• Analysis: Analyzes processes and creates deliverables to provide information and recommendations for improvements. Breaks down problems and issues into sub-components and assesses the costs, benefits, and risks of various options. Is able to select and recommend the best solution based on a thorough examination of all considerations. Is able to explain and justify actions in a systematic and logical fashion.
• Communication: Shares and receives information using clear oral, written, and interpersonal communication skills. Demonstrates effective written and oral communication skills. Actively listens, provides constructive feedback, and demonstrates respect for differing views. Tailors communications to diverse audiences.

Physical Demands

Medium to Heavy Physical Demand: The job requires constant standing, frequently walking, and bending/stooping. Proficient communicative, auditory, and visual skills. Attention to detail and ability to write legibly. Ability to lift/push/pull 100 lbs. For any weight over 35 lbs., use Safe Handling Equipment. The base compensation range for this role is $40.36 - $66.84 hourly. Individual compensation will be determined by the selected candidate's qualifications, previous work experience, and/or education. Please note that the pay range does not include any applicable incentive compensation programs such as shift differentials, clinical ladders, and other incentives that may be available for those in eligible positions.
