Per Diem Home Health Nurse Practitioner (NP) - Robbins, Tennessee

This dynamic per diem opportunity offers ultimate flexibility, allowing you to create your own schedule with shifts available any day of the week. There is a minimum hourly commitment, which may be fulfilled weekly or bi-weekly. Assignments are located throughout Fentress County, including Jamestown, Allardt, and Grimsley.

Responsibilities:

• Conduct comprehensive home health risk assessments for Medicare Advantage plan members
• Assess patients current and prior medical history, conditions, and medication regimens
• Provide in-depth education to patients about their health conditions and prescribed therapies
• Identify gaps in care and generate recommendations to close these gaps
• Carry out either annual wellness assessments or post-discharge visits as required
• Note: This role does not include creating treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license in Tennessee
• Board certification through AANP or ANCC
• MSN degree from an accredited nursing program
• Current BLS CPR certification
• At least 6 months of experience as a practicing NP is preferred
