Job Opportunity

Position Overview

This position is responsible for coordinating all aspects of care for transplant and disease management patients, including both pre-operative and post-operative assessments and discharge planning. The role requires interaction with various departments to facilitate comprehensive patient care.

Key Responsibilities

• Coordinate inpatient and outpatient care per program assignment, including record review, medication refills, and prior authorizations.
• Provide patient education regarding care plans and medications.
• Assist with patient discharge planning as necessary.
• Make referrals for medically necessary services including consultations and equipment.
• Act as a liaison between clinical staff, administrative staff, and physicians within the transplant department.
• Support patient admissions and discharges as required.
• Carry out patient care orders as directed by physicians in accordance with hospital policy.

Requirements

• BLS Certification
• Current CA Nursing License
• Bachelor's degree in a related field
• Experience with EPIC Phoenix for outpatient services is mandatory.

Preferred Qualifications

• Minimum of 2 years of relevant experience
• Experience in transplant patient populations is highly preferred
• Knowledge of cardiology and medication reconciliation processes
• ACLS Certification

Work Schedule

This role operates on a Monday to Friday schedule, 5 days a week, with no weekends or holidays required. Standard hours are from 8:00 AM to 3:30 PM.

Additional Information

• Successful candidates must complete labs and onboarding requirements within specified timelines.
• Work history verification covering the last 7 years is required.
• Participation in educational programs about transplantation to various groups in the community is expected.
