Guided by our core values of Passion, Respect, and Excellence, the Memory Care Activity Coordinator is responsible for developing and leading specialized programs that promote cognitive stimulation, emotional well-being, and resident-centered experiences.Responsibilities for Memory Care Activity Coordinator:Coordinate and oversee the development of unit objectives, policies, and procedures for the Memory Care program.Prepare and present records and reports to the Executive Director regarding program budgets, progress, and goals.Conduct admission and discharge assessments and develop individualized plans of care.Develop and implement engaging daily programming designed to support residents' cognitive, emotional, physical, and social well-being.Collaborate closely with staff, residents, and families to ensure open communication and personalized care experiences.Pursue continuing education in dementia care and related fields to enhance program quality and best practices.Lead, motivate, and support a team of compassionate caregivers to deliver exceptional resident experiences.Maintain a therapeutic, structured, and enriching environment that promotes dignity and quality of life for residents with memory impairment.Qualifications for Memory Care Activity Coordinator:An active CTRS certification in the state of PA is required.Prior recreation or activities experience in a nursing home or Memory Care setting preferred.Demonstrated knowledge and skills necessary to provide care appropriate to the age-related needs of residents served.Ability to communicate and interact tactfully with residents, family members, staff, visitors, government agencies, and the public.Strong organizational, leadership, and interpersonal skills.Passion for working with seniors and supporting individuals living with dementia and memory-related conditions.Our Core Values in Action:Passion – Creating meaningful programs that bring joy, engagement, and purpose to residents living with memory impairment.Respect – Honoring each resident's life story, preferences, and individuality with compassion and dignity.Excellence – Delivering innovative, person-centered programming that promotes well-being and enhances the resident experience.Benefits for Memory Care Coordinator:Tuition reimbursementEmployee referral bonusHealth, vision, and dental benefits401(k) with matchEmployee engagement and culture committeeCompany-sponsored life insuranceEmployee assistance program (EAP) resourcesJoin our team at Oxford Rehabilitation and Healthcare Center, a 179-bed Sub-Acute, and Long-Term Care facility where compassion and quality care are at the heart of everything we do.
