Clinical Supervisor: Physical Therapy

What You’ll Do Ensure patient care is coordinated and managed appropriately. Serve as the first point of contact for field clinicians, providing timely support. Supervise clinical personnel and oversee patient care delivered in the home. Work in a hybrid model: two days in the Walnut Creek office and three days in the field (12 patient visits per week, plus supervisory observations). Coordinate training and educational programs for nurses in the field to improve clinical outcomes. Assess staff learning needs and formulate specific plans and goals for the next evaluation period. Act as the primary contact for clinician escalations and patient care issues. Coordinate bi‑weekly case conferences to discuss progress and revise care plans. Review and evaluate each case, guide clinicians to improve performance and quality of care. Review patients’ clinical diagnoses, medications, procedures, and courses. Assist clinicians in establishing immediate and long‑term therapeutic goals and care plans. Qualifications At least two years of management or supervisory experience in a rehabilitation therapy department or program, preferably home care. Degree (bachelor’s or master’s) in an applicable discipline from an accredited institution. Current California Physical Therapist license. Current Basic Life Support (BLS) certification. Valid California driver’s license with an acceptable driving record. Must maintain all licenses and certifications current at the time of hire and throughout employment. Mandatory Field Mandate All positions must involve mandatory field visits to patients’ homes; remote or work‑from‑home positions are not available unless explicitly specified. Compensation & Benefits Salary range: $143,000—$163,000, plus quarterly bonus. Starting pay is commensurate with relevant experience above the minimum requirements.
