Nurse Practitioner

Position Summary Providence at Home with Compassus seeks a compassionate and experienced Nurse Practitioner to provide essential support to patients and families across various care settings, including homes, long‑term care facilities, and hospitals. The Hospice and Palliative Care Nurse Practitioner (NP) works independently as an advanced nursing professional, coordinating and assessing care needs, creating and modifying care plans, and making referrals in collaboration with the interdisciplinary team. Responsibilities Comply with organization‑wide policies and with all regulatory agencies’ requirements. Create a safe environment for patient care and promptly address any unsafe conditions. Under appropriate physician supervision, conduct thorough medical, functional, and psychosocial assessments of assigned patients. Perform and interpret diagnostic tests and procedures as defined by protocol. Initiate and modify drug therapies, including opioid prescribing, per approved protocols or direct physician orders. Perform face‑to‑face, end‑of‑life, and recertification assessment visits. Collaborate with the health team to ensure continuity of care and provide patient information to the physician. Order drugs, supplies, IV therapy, antibiotics, and opioids, including adjusting dosages as needed. Order feedings and manage placement/replacement of nasogastric or gastric tubes, adjusting rate and formula as required. Assist families and patients with decision making regarding goals of care, withdrawal or withholding interventions, and resuscitation status. Educate patients and families about diagnosis, prognosis, and palliative care concepts. Assist in patient referrals to appropriate health facilities and community resources. Educate nursing staff, nursing facility departments, and family members regarding patients’ special needs. Maintain courteous and cooperative relationships with patients, families, and public personnel. Participate in the development and implementation of evidence‑based educational initiatives. Document clinical notes accurately and in a timely manner per organization policies and regulatory requirements. Utilize the Hospital Information System and other computer‑based communications. Participate in the provider on‑call rotation. Perform other duties as assigned. Serve as the primary link and resource for patients, families, and staff for hospice and palliative care education. Maintain schedules and productivity standards and document data on all patients referred and seen. Participate in quality improvement activities related to hospice and palliative care. Education and Experience Master’s Degree in Nursing, required. Minimum of two (2) years’ experience in an acute or transitional care setting, required. Certifications, Licenses, and Registrations Active and unencumbered RN license in the state of practice, required. Upon hire: National Provider BLS – American Heart Association (Vendor‑Managed), required. Physical Demands and Work Environment The role requires regular standing, walking, and manual dexterity, with the ability to lift and move objects up to 50pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and focus adjustment. Exposure to bodily fluids, infectious diseases, and clinical equipment is anticipated. Compensation Base pay range: $56.93–$73.67 per hour. Additional compensation may be available, including shift differentials, standby/on‑call, overtime, premiums, extra shift incentives, or bonus opportunities.
