Licensed Clinical Social Worker Location: NHC Home Care Merritt Island Pay: $40 - $47 per regular visit (based on experience) Position: Medical Social Worker Responsibilities Supervise and evaluate the social and emotional needs of patients related to their medical conditions. Provide counseling based on evaluation findings. Deliver direct social services aimed at restoring the patient's optimal social and health adjustment. Assess social and emotional factors to minimize their impact on patient's health and coping ability. Assist patients and caregivers in understanding, accepting, and following medical recommendations. Help patients and caregivers address personal and environmental difficulties that may interfere with medical care. Identify resources such as caregivers and community agencies to support patients resuming community life, including discharge planning. Work with providers and the home care team to understand significant social and emotional factors related to health problems. Participate in developing the plan of care. Qualifications Master’s Degree from an accredited school of social work (Council on Social Work Education). At least one year of social work experience in a health care setting.
