Details Work within a 20-25 bed unit covering Cardiology, Nephrology, Trauma Surgery, Ortho Surgery, Vascular Surgery, Neurosurgery, GI Surgery, Urology, Transplant, and ICU/Stepdown services Day shift schedule from 8:00am to 5:00pm 36 hours per week over 3 days per week No on-call requirements No weekend requirements No holiday requirements Uses Epic EMR for documentation and care planning Requirements Active RN license (NC or compact) required 2 years of Case Management or Care Management experience in an acute care setting required 2 years of healthcare experience as a Registered Nurse required Nursing Diploma, ADN, or ASN from an accredited nursing school required; BSN or MSN preferred Strong assessment and critical thinking skills required Additional Information Provide comprehensive assessment, planning, implementation, and evaluation of individual patient care needs Coordinate discharge planning and care progression for patients with complex medical conditions Participate in daily Care Management Touchpoints and CAPP meetings to review patient progression and discharge readiness Attend weekly Complex Care Meetings to collaborate on complex patient cases and identify trends Consult with Social Work and Utilization Management per departmental protocols Coordinate referrals to community resources and post-acute care providers Communicate with patients and families regarding medical milestones and discharge plans Experience in an educational or teaching hospital setting is beneficial American Traveler Job ID P-749433.
