Licensed Clinical Social Worker (LCSW) – Full-Time

Position Overview

We are seeking a compassionate and dedicated Licensed Clinical Social Worker (LCSW) to join an interdisciplinary behavioral health team serving residents in long-term care facilities throughout the Chicago area. This role offers the opportunity to provide high-quality mental health services to older adults and individuals with complex medical needs in a collaborative, patient-centered environment.

This position offers a predictable Monday through Friday schedule, professional autonomy, and strong clinical and administrative support.

Key Responsibilities

Clinical Care

• Provide individual, family, and group therapy services.
• Conduct psychosocial assessments, screenings, and treatment planning.
• Support patients experiencing depression, anxiety, grief, trauma, adjustment disorders, substance use concerns, and other behavioral health conditions.
• Implement evidence-based therapeutic interventions and behavioral management strategies.

Care Coordination & Collaboration

• Collaborate with nursing staff, physicians, rehabilitation teams, and other healthcare professionals.
• Participate in interdisciplinary care planning and case reviews.
• Contribute to improving resident outcomes through coordinated behavioral health services.

Documentation & Compliance

• Maintain accurate, timely, and compliant clinical documentation.
• Adhere to ethical, professional, and regulatory standards.
• Complete assessments, progress notes, treatment plans, and required documentation according to established guidelines.

Facility Partnership

• Build strong professional relationships with facility staff and leadership.
• Serve as a trusted behavioral health resource for residents and care teams.

Qualifications

Required

• Active, unrestricted Licensed Clinical Social Worker (LCSW) license.
• Master's degree in Social Work (MSW) from an accredited program.
• Valid driver's license and reliable transportation.
• Strong interpersonal, written, and verbal communication skills.
• Ability to work independently and collaboratively within an interdisciplinary team.

Preferred

• Experience in long-term care, skilled nursing, geriatric care, behavioral health, or related healthcare settings.
• Experience providing psychotherapy, assessments, and treatment planning.
• Knowledge of behavioral health conditions commonly affecting older adults.
